Joining Is Simple
Exclusions & Limitations
- This is not a dental insurance plan.
- Cannot be used with another plan or insurance; for services for injuries covered under workman’s compensation.
- Cannot be used for treatment which, in sole opinion of the treating dentist or doctor, lies outside the realm of their capability or for referrals to specialists.
- Not for hospitalization or hospital charges of any kind or for costs of dental care which is covered under automobile, medical, or other insurance.
- Interest free financing for 6 months & up. Available for treatment cost over $999.
- Financing cannot be used to pay for the plan or in conjunction with savings from this plan.
- No premium refunds will be issued at any time if participant decides not to utilize dental plan.
- Plan is currently only honored at ApolloniaDental.
